Officer Information Entry Form

 

Report Officers Information:   This is required to allow State & National PTA to communicate with each PTA and the leadership team responsible.  Officer’s information is required yearly even if there are no changes to officers.

 

PTA 2022-2023 Officers are due to CT PTA by July 1st.

Even if you have no change in officers as of July 1, please submit an updated Officer Information Sheet.   

 

Please note, officers information is shared with National PTA.

When a vacancy is filled, or an officers information is changed, or an officer resigns, etc, report all changes here.
